v4/system/blocks.c:96-122 and :159 push results directly: BLOCK and BUFFER net +1, UPDATE and SAVE-BUFFERS +1, none checking for room. On a full stack the oldest value is overwritten silently instead of raising the overflow fault. Found in the 2026-11-02 review.
